New Hampshire Transition Community of Practice

Description

The New Hampshire Transition Community of Practice promotes a way of working that fosters cooperation and collaboration among all partners involved in transition to life after high school and in making a difference in the lives of New Hampshire’s youth, particularly those with disabilities.Characteristics of a Community of Practice:

Ø  Non-hierarchal and Voluntary

Ø  Varied stakeholder representation

Ø  Groups of people united over a shared passion, issue(s) or experiences

Ø  Convene on a regular basis

Ø  Focus on relationships and outcomes

Ø  Organic in its development

Ø  Always asking who is missing

Ø  Open to varied perspectives

Ø  Work collaboratively to share, plan and take action

Ø  Finding ways to connect the work at the local, state, and national level

 

To join the NH Transition Community of Practice click “Join the NH Transition COP” up in the upper right hand column.   To view or join other NH work groups scoll down page until you see groups listed in right hand column and click on one you are interested in. Follow instructions if you wish to join that group.
