The Practice Group on Health believes that health is part of who we are as people and must be considered in all aspects of an individual's transition from childhood to adult life. From the World Health Organization definition--"Health is not only the absence of infirmity and disease but also a state of physical, mental and social well-being."
The Practice Group on Health is composed of members representing hospitals and clinics, Regional Centers for Children and Youth with Special Health Care Needs, Parent to Parent, Family Voices of Wisconsin, schools, the Department of Health Services, the Department of Public Instruction, and more.
Activities for the practice group include: Resource mapping of best practice health-related resources, a healthcare checklist and pocket guide, healthcare transition curriculum, and environmental scan of youth health transition activities in Wisconsin and nationally.
