Pittsburgh PYT Project


Welcome to the Pittsburgh Public Schools/Allegheny County Systems of Care Initiative - Partnership with Youth Transition Project
The purpose of this project is to develop an individualized, cross-agency planning process to assist youth and young adults with emotional support and mental health needs to successfully transition to adult life and services. The Allegheny County PYT Project is one of five nationally funded through a collaboration between the US Department of Education and the US Department of Health and Human Services/Substance Abuse and Mental Health Services Administration. The other four projects are from: Utah, Maine, Clark County, Washington and Minnesota.

The mission of PYT is to "empower youth/young adults, consumers, families and communities to manage and advocate for their needs and realize their dreams." The goal of this project is to help young adults with mental health needs to achieve their full potential by providing access to education, support, and relevant resources so that they may live, love, learn, play, and recover.

The population of youth served by this project include:
-- youth and young adults ages 14-21 (age out at 25)
-- experience serious emotional disturbance/serious mental illness
-- have multi-system involvement
-- experience functional impairment affecting home, school, work, or community life
